Introduction Outcomes Based Agreements (OBAs) are financial arrangements that offer the opportunity to align payment health outcomes in real-world, and share risk by providing long-term solutions grant access medicines, with reimbursement only when performance is achieved. OBAs most likely be useful there high uncertainty clinical data, but they difficult design implement, other options usually preferred payers. As a result, have been more exception than norm, not clear pattern indicates if an OBA succeed practice. Methods Through retrospective exercise NHS Wales (Project IDEATE: Innovation Data Evolve That Enhance patient outcomes), we explored circumstances under which might appealing payers, assessed implementation challenges solutions, propose framework evaluate feasibility of medicine for OBA. Results Along mitigating some uncertainties associated lack mature data at time launch, must also consider factors: commercial viability agreement, administrative burden, its cost implementation. Also, Health System commitment Value-Based Healthcare agenda and, importantly, willingness sustainable optimise treatment, key support this approach. Practical considerations include: how relevant going selected tracked whole model fit within current procurement finance infrastructures, industry works collaboration System. Conclusions Insights from Project IDEATE will used explore our applied future.
